To obtain approval to operate as a food vendor
at Temporary Events, you must:
1. Call and make an appointment for
a personal interview with a health inspector.

2. Have a Colorado Sales Tax ID# for Special (or
multiple) Events, or Mobile Unit (Colo Dept of
Revenue)
3. Complete a JCDHE Form 3610 bring with you.
Or, you can fill one out at the Jefferson County
Health Department on Look Out Mountain in
Golden.
